The NHS Confederation has asked people if they want cake and seem encouraged that the answer is yes. In the confederation's parallel universe people can (1) have their cake and eat it, (2) it will be better cake than before, (3) they won't need to wait for it, and (4) it will be free! They are shrewd enough to design a survey that can support their bias. No, the truth is that some people need more cake than others and usually it is not the ones who are asking for it the loudest. The NHS has been one of the best cake makers in the world (and still is world class at giving access to cake) but it has had its funding throttled since 2009 and subject to reorganisations that were 'visible from space'. The answer to this for the UK (in my personal opinion) is collaboration in healthcare (not competition), a long-term (more than 5 year) funding agreement by government for the NHS, and a delegation of targets and management to people who are trained to do it (doctors, nurses, and scientists) not politicians.
